Patient Forms
Please print and fill out these forms so we can expedite your first visit. There will be a total of 4 forms that print out. Fill them out completely and bring them with you to your appointment.
As of November 1,2009, a photo ID will be required at each office visit to verify patient identity. Please bring your photo ID with you to each and every appointment. We comply with the Identity Compliance Program. Thank you for your cooperation.

Our address is: 1268 East Broad Street Suite #1 Elyria, OH 44035
Take Route 57 South to East Broad Street, turn left on East Broad/Sugar Ridge Road. Proceed approximately 1/4 mile down the road. The Liberty Point Building will be on the left. We are the first suite closest to the road.

Take Route 83 South to Sugar Ridge Road. Turn right on Sugar Ridge Road. Proceed West for 2-3 miles. When you reach the Elyria city limits the Liberty Point building will be on your right.
Or
Take 480 to the Route 10 split and proceed West to Route 57 North, proceed North to the second traffic light and turn right on East Broad/Sugar Ridge Road. Proceed about 1/4 mile down the road. The Liberty Point Building will be on the left. We are the first suite closest to the road.

Take St Route 20 West to State Route 57,turn right or South to East Broad Street,turn left on East Broad/Sugar Ridge Road. Proceed about 1/4 mile down the road. The Liberty Point Building will be on the left. We are the first suite closest to the road.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Make sure to have all paperwork completed, have your current insurance card and photo ID available, and your co-payment.
